A sticky button, up to five questions, and every submission counted per member on a live calendar.

A form is a button that stays at the bottom of a channel. A member with the right role presses it, answers a few preset questions in a pop-up, and that is one submission: logged to a channel, counted for the person who sent it, and drawn on a calendar on the dashboard. It was built for outreach logs (which server did you contact, who, what did they say), and it works just as well for shift reports, bug reports or a short application.

The first sentence makes the whole thing in one go and only asks where to log the answers. A bare set up a form asks for the name, the channel, the role, the questions and the log channel one at a time. Say the outreach ones at the questions step for the four above. Start with dispresso or mention it. It asks for anything it still needs, writes the same settings the dashboard writes, and the whole list of sentences is on Talk to Dispresso.

What a form holds#

TitleThe name on the button's message and in the log. Also the key you say in chat.
ChannelWhere the button lives. It stays the newest message there: the bot reposts it under new chat and deletes the old copy.
RolesWho can press it. Empty means anyone. People with Manage Server always can.
QuestionsUp to five, because a Discord pop-up holds five. Each is short or long, required or not, with an optional hint.
Log channelEvery submission is posted there as an embed: who, when, every answer. Optional; the dashboard keeps them either way.

Five questions is Discord's limit, not ours

A modal holds five text inputs with 45 character labels. Ask for more and the bot says so rather than dropping one. Need a longer questionnaire? A ticket type can ask in DMs with no cap.

From the dashboard#

Dashboard, your server, Forms. The page lists your forms, edits their questions, roles and channels, and shows the newest submissions. Saving a form with a channel posts the button straight away; Post puts it up again if it was deleted or moved.

    Calendar. One number per day, in your own time zone. Click a day to read every submission from it. It refreshes on its own every few seconds, so a form sent in Discord appears without a reload.

    Per member. Today, the last seven days, this month and all time for everyone who has sent something, most this month first.

    Filters. With more than one form, the calendar and the list switch between all forms and one.

stats is the same today / 7 days / month / all time table, per member, as an embed. All four need Manage Server.

The developer API reads forms and their submissions with the forms:read scope, so a sheet or a bot of your own can track the same numbers. See the Developer API guide.
